Will work in the Community Support Program and would include home visits to a continuum of living options including Supervised Group Living homes (SGL) ,Semi-independent and Independent living options. Must be able to function independently in conformance with organizational policies, procedures and Ethical Standards as well as local, state and federal regulations. Must be able to document and communicate orders accurately in a timely manner. Must be able to maintain a clean, safe and functional working environment. Provides accurate and timely information using the utmost discretion and protecting confidentiality of information. Interact with assigned patients throughout the shift both informally and in structured activities.

May assess crisis situations and determine the need for intervention. Employee shall function in compliance with all organizational policies and procedures as well as local, state, and federal regulations and observe Ethical Standards and Confidentiality Standards at all times.

Will be responsible for intake interviews, diagnostic assessments, and provide individual and group therapy with a special emphasis on Evidence- based practices. Fulfill the responsibility of “Team Leader” for a specialty treatment team made up of the Psychiatrist, Nurse, Skills Trainers and Medical Assistants. Fulfill the responsibility of case coordinator insuring that continuity of care is provided each client. Maintain complete, detailed service records to insure quality care for each client. All Master’s degreed clinical employees participate in Access Center coverage.

This can include but not limited to: (a) meeting with the client on a scheduled basis to assess current functioning and to teach skills, (b) providing assistance on behalf of the client, as needed in linkage with entitlements, service agencies and the medical community and (c) jointly developing a treatment plan and regularly assessing progress in achieving those goals.
